Medical history information is updated at the time of check-in using our in-office kiosks (iPads).
- Patient consent forms
All patient consent forms are sent prior to your first appointment.
- Arrival times
Our reminder messages ask:
• New patients to arrive 20 minutes early
• Follow-up patients to arrive 10 minutes early.
