Payment Policy & Patient Responsibilities

It is the responsibility of each patient to verify their insurance coverage/medical benefits with their insurance company. It is the responsibility of the patient to understand that insurance authorization for services does not guarantee payment.

It is very important that patients do this prior to receiving any services recommended/referred by our physician. This includes, but is not limited to, orthotics, second opinions, radiology services (MRI, CT, etc.), surgeries/injections or physical therapy.

Medical Supplies

Following your providers evaluation, certain medical supplies may be recommended as course of treatment.

Medical supplies may fall into the durable medical equipment category with your insurance company. It is the patient’s responsibility to know their insurance benefits, as they may be limited or nonexistent.

As a health care facility, we are not able to take back medical supplies once dispensed.

Outside Diagnostic Exams

Your provider may order an outside exam e.g. MRI, CT, etc. Please keep in mind each insurance plan has different prior authorization guidelines. However, if you have not heard from our authorization department within seven days call and speak with Tina, at 503-905-4143.

As soon as your exam has been schedule please call our scheduling department at 503-656-0836 and set up an appointment to go over the exam with your provider.

Managed Care Plans

We are a participating provider with most major health care and workers’ compensation plans.

Email Communication Terms of Service

In these Terms of Service, all references to ROC shall include ROC and any person or entity acting on behalf of ROC to deliver email communications.

By subscribing to our email communication, you understand and agree to the following terms of service for email communication:

  • I hereby consent to receive communications about events that I have expressed an interest in attending via email and about other events and information that ROC determines might interest me based on prior expressed interest.
  • I understand that ROC is committed to keeping my email address confidential and will not sell or lease my email address to anyone.
  • I understand that ROC will only share or distribute my email as allowed or required by law.
  • I understand that email communications, outside of the ROC Portal messaging, may contain information I wish to keep confidential, such as information about my health care.
  • I understand that the email messages sent by ROC, outside of the ROC Portal, may not be encrypted and may not be confidential.
  • I also understand that ROC cannot assure the privacy of an email. Email travels via the public Internet or my email carrier’s network. It is not possible to verify that an email is actually received, opened and read by the intended recipient. I accept the risk that my medical information may not be confidential when it arrives at my email inbox.
  • I agree and understand that ROC takes no responsibility for and disclaims any and all liability arising from any breach of confidentiality not caused by ROC, inaccuracies or defects in software, communication lines, virtual private network, the internet or my internet service provider, mobile carrier or mobile carrier’s network, access system, computer hardware or software, or any other service or device that I use to access emails.
  • I agree and understand that ROC reserves the right to terminate email communication with me at any time for any reason without prior notice.
  • I understand that I may terminate receiving email communications by opting-out at any time. To opt-out, I may click the “Unsubscribe” or “Manage Subscriptions” link in the footer of any ROC email that I receive outside of the ROC Portal, or “Reply” to the sender of the email directly to request to unsubscribe.
  • I agree to notify ROC as soon as practical if my email address changes. I also agree and understand that ROC takes no responsibility for and disclaims any and all liability arising from my failure to notify ROC of a change in my email address.
  • I understand that I may receive emails on my mobile phone or such other device or program that allows the receipt of emails. By providing my consent to participate in this service, I approve any charges from my mobile carrier or other third party to receive such emails.
Text Messaging Terms of Service

In these Terms of Service, all references to ROC shall include ROC and any person or entity acting on behalf of ROC to deliver the ROC Text Messaging services. I understand and agree to the following terms of service:

  • By providing ROC with my mobile phone number, I expressly consent to receive text messages including reminders, referrals and/or waitlist notifications from ROC sent to the mobile phone number provided.
  • I understand that an automatic dialing system may be used to deliver text messages to me. I expressly consent to ROC to use an automatic dialing system to deliver text messages to the mobile number that I have provided to ROC for contact purposes. I understand that text message communications from ROC may contain information I wish to keep confidential, such as information about my healthcare team, date of appointment and location of service.
  • I understand that ROC text messages are not encrypted and ROC cannot assure the privacy of an ROC text message. Text messages travel via the public Internet or my carrier’s network. It is not possible for ROC to verify that a text message is actually received, opened and read by the intended recipient. I accept the risk that my medical information may not be confidential when being sent via text message.
  • I agree and understand that ROC takes no responsibility for and disclaims any and all liability arising from any breach of confidentiality not caused by ROC, inaccuracies or defects in software, communication lines, virtual private network, the internet or my internet service provider, mobile carrier or mobile carrier’s network, access system, computer hardware or software, or any other service or device that I use to receive or access text messages.
  • I understand that ROC reserves the right to terminate its Text Messaging services at any time for any reason without prior notice.
  • I understand that I may terminate my participation in ROC Text Messaging services by texting STOP at any time in response to an ROC text. In such a case, I understand and agree that I may receive one final text message confirming that the ROC Text Messaging service has been stopped.
  • I agree to notify ROC as soon as practical if my mobile number changes. ROC is not responsible for any liability arising from my failure to notify ROC of a change in my mobile number.
  • ROC does not have a separate charge for this service; however, I understand that message and data rates may apply from my mobile carrier and I am responsible for any such charges.
